Please write

For the Ukranian language Wikipedia please visit http://uk.wikipedia.org; for Wikimedia Ukraine please visit http://ua.wikimedia.org

I do not think you like abovementioned text on the top of the main page. So I would suggest to change the name of Wikimedia UK site to http://gb.wikimedia.org in accordance with http://en.wikipedia.org/wiki/ISO_3166-1

Then on the page http://uk.wikimedia.org/ the disambig text could be created with references to:
